Release of Liability Form
REQUIRED for ALL Players
All players must submit a completed Release of Liability Form signed by each player’s parent. Original signatures only will be accepted, no copies, please. These forms must be submitted at team registration on Friday, September 4th. Failure to do so will result in the inability to participate in the tournament. Click HERE to access the form.
